Abstract
The MOMENT project aims to transform maternity care by promoting sustainable, primary care-based midwifery-led models, ensuring continuous care, enhancing maternal health outcomes, reducing unnecessary obstetric interventions, and optimising healthcare resources - contributing to Sustainable Development Goals 3 and 5. The project focuses on bridging the gap between evidence and real-world implementation of midwifery-led care by applying a co-designed, mixed-methods approach rooted in equity, diversity and sustainability. Despite strong evidence supporting its benefits, global adoption of midwifery-led care remains inconsistent, with many countries facing barriers to effective implementation.
A multidisciplinary consortium spanning Belgium, Canada, Portugal and Spain will lead participatory research to addressing systemic, organisational, and professional barriers through five work packages. These include the development of evidence-based tools like implementation plans and patient journey maps to guide policymakers and healthcare providers in adopting midwifery-led care and evaluating its impacts. The project includes a series of studies, including mapping existing midwifery-led care models, cost analysis, and synthesising evidence on policies that promote successful implementation. Diverse end-users, including women, families, professionals and policy will be engaged through co-leadership and stakeholder panels. Social sustainability and cost evaluation modelling will be integrated to support long-term viability. A holistic, person-centred, and multidisciplinary approach to ensure continuity of care from pregnancy to early postnatal life, integrating midwifery services within existing primary healthcare systems underpin the project. MOMENT will help shift from fragmented, risk-oriented medical-led care to a more integrated, community-oriented model, of perinatal care, as a sustainable, scalable, and effective part of healthcare systems across Europe and beyond.
